Amateur athletes and tobacco: what you really gain when you quit smoking
Smoking and doing sport? Tobacco strips 10% off your VO2 max and blocks recovery. Here is what your body takes back from the moment you quit.
The scientific basis on quitting smoking was reviewed on a voluntary basis by Pr. Bertrand Dautzenberg , a tobacco specialist, in order to rule out gross, potentially dangerous errors. It reflects positions commonly shared by health professionals and health agencies, without always corresponding exactly to his thinking or his practice. He is not the author of this text; he has only carried out a vigilance review of it.
You run, you swim, you play Sunday football, and you smoke 5 or 10 cigarettes a day. You tell yourself it balances out, that sport "wipes" the cigarette. Spoiler: no. The good news is that from the moment you quit, your body pays you back fast — much faster than you think.

1. The carbon monoxide trap
The carbon monoxide (CO) from tobacco combustion has 200 times more affinity for haemoglobin than oxygen. Result: on your red blood cells and in your muscles, more than 10% of the space is taken by CO instead of oxygen.
How can an athlete, even a weekend one, reach their performance if they already have a 10% reduction in oxygenation? CO holds them back considerably, and they hit their VO2 max much earlier than a non-smoker.

2. The heart works harder, for nothing
Even before you run, your resting heart rate is higher than a non-smoker's. And right after a cigarette, it spikes further. On exertion, your heart compensates the oxygen shortfall by beating faster — so you tire sooner and recover less well.
3. Recovery is slower
You get more muscle soreness, more post-exercise pain, and you take longer to chain two sessions. Confirmed by every study: smokers have a significantly degraded recovery index.
4. More injuries and cramps
Tobacco weakens connective tissue (tendons, ligaments) and impairs muscular microcirculation. Result: more tendinitis, more cramps, more injury risk, especially in endurance sports.

- 8 hours CO level halved.
- 24 hours CO fully cleared. Your red blood cells transport oxygen at 100%.
- 2 weeks improved circulation and muscle capillarisation.
- 3 months VO2 max significantly back up. Endurance and recovery markedly better.
- 6 to 12 months performance level of a non-smoker, at equivalent training.

If I only smoke at the weekend, does it affect my sport?
Yes. One evening with 10 cigarettes on Saturday will weigh on your long run on Sunday: CO level still high (clearance ~24 hours), circulation disrupted for 24-48 hours. Many "occasional weekend" smokers underestimate this point. -
How long before my performance is back to normal?
CO clears in 24 hours: you will feel the difference in the first week. VO2 max climbs back significantly in 3 months. Non-smoker level at constant training: 6-12 months. -
Will sport make me put on weight if I quit smoking?
The opposite: sport limits the quitting weight gain. Sporty ex-smokers gain on average 2 kg less than sedentary ones. -
Should I quit before or after a major race?
If the race is in less than 3 weeks: tricky, withdrawal can upset mood and sleep. Aim for after. If it is in more than 3 months: quit now, you will perform better on race day. -
I smoke just before or during exertion — is it that bad?
Yes. The cigarette before effort sharply raises heart rate, blood pressure, and the perceived effort. During effort: it is physically very unfavourable, and increases cardio risk.
