The scientific basis on quitting smoking was reviewed on a voluntary basis by Pr. Bertrand Dautzenberg , a tobacco specialist, in order to rule out gross, potentially dangerous errors. It reflects positions commonly shared by health professionals and health agencies, without always corresponding exactly to his thinking or his practice. He is not the author of this text; he has only carried out a vigilance review of it.

A cigarette urge that hits mid-quit is a wave. Strong, destabilising, but short — it lasts 3 to 5 minutes max. The 4D method gives you four moves to chain together so you can ride through without going under.

Why a craving only lasts 3 to 5 minutes

When you feel the urge rise, it is your brain demanding nicotine. That spike, called a craving in science, is a short peak. Addiction research shows that without a cigarette to feed it, it dies down on its own within a few minutes.

3 to 5 min the average duration of an unsatisfied craving. You do not have to "hold on all day" — you just have to hold on 5 minutes, then start over.

The trap: those 5 minutes feel endless when you are inside them. That is exactly when the 4Ds come in.

The 4Ds, how-to

  1. Delay

    push the urge back by 5 minutes. "I will see in 5 minutes if I really want to." Often the urge fades on its own before the timer is up.

  2. Distract yourself

    busy your brain elsewhere: open an app, put on a song, call a friend, read a few pages, cook.

  3. Drink water

    slowly sip a tall glass of cold water; it activates the vagus nerve and gives your hands and mouth something to do.

  4. Deep-breathe / Decide

    re-read (in your head or on your app) why you are quitting. Reasons list, letter you wrote yourself, a photo that drives you. Decide again, freely.

The trick: you do not need to do the 4Ds in order. If you are at work, distract yourself or decide. If you are at home, get moving. Adapt to context.

Customising your 4Ds

  1. List your 5 favourite distractions

    a specific song, a mobile game, podcast, series, comic, chat with a loved one, drawing… This will be your emergency kit.

  2. Spot 3 places to move

    your hallway, the block, the staircase at work. The easier to access, the better.

  3. Write your 3 real reasons for quitting in a phone note. Not the social reasons, yours. Reread them at the fourth D.

  4. Build a breathing routine

    4 seconds in, 4 seconds hold, 4 seconds out, 4 seconds hold. One minute is usually enough.

What if a craving still overwhelms you?

It will happen, especially in the first days. That is normal. A few reminders:

If nothing works, something may be missing

If the 4Ds are not enough, that is not a failure — it is a signal. You may need:

  • A nicotine substitute or a medication (varenicline, cytisine) — the craving is too strong.

  • A stop smoking specialist consultation — an emotional trigger needs therapeutic work.

  • A strategy adjustment — perhaps you tried to quit at a bad moment (too much stress, a major life event).

  • How long before cravings really get rarer?

    The hardest peaks are between Q3 and Q7. By Q14, it is much better. By Q28, the worst is over. After 3 months, urges are rare and mild.
  • If I slip during a D, do I have to start over?

    No, you have already won time. A one-off slip does not erase your progress. You pick up the 4Ds at the next urge. See our article on slips.
  • Do the 4Ds also work for vaping?

    Yes, but usage is different: the vape being a harm-reduction tool, you often use it as a "bonus D" (Decide to vape instead of smoke) before aiming for full cessation later.
  • I have heard of the "5Rs" and other methods — better?

    The 5Rs are a framework to prepare quitting (Relevance, Risks, Rewards, Roadblocks, Repetition), not to manage a craving in real time. The 4Ds remain the operational method most widely taught and easiest to remember.
  • Do I have to do all 4 Ds systematically?

    No. Often one is enough — especially Delay. The 4Ds are a toolkit, not an obligation.
