Snus and nicotine pouches: harm reduction, the Scandinavian context and EU legislation
Sweden has 44% fewer tobacco-related deaths thanks to snus. But snus is not harmless. The truth about oral products and the new pouches.
The scientific basis on quitting smoking was reviewed on a voluntary basis by Pr. Bertrand Dautzenberg , a tobacco specialist, in order to rule out gross, potentially dangerous errors. It reflects positions commonly shared by health professionals and health agencies, without always corresponding exactly to his thinking or his practice. He is not the author of this text; he has only carried out a vigilance review of it.
You have heard of Swedish snus. You have seen an athlete with a pouch under their lip. Or you have noticed adverts for ZYN, Velo and On! everywhere in the past two years. You wonder what these products really are.
Honest answer: less risky than cigarettes, but far from harmless. And the nuance matters.
Snus, pouches: what are they exactly?
Snus is a moist pasteurised tobacco packed in small sachets, tucked under the upper lip. Nicotine and other compounds are absorbed through the mouth lining. No combustion: no smoke, no tar, no carbon monoxide.
Modern nicotine pouches (Velo, ZYN, On!, Nordic Spirit) look similar to snus but contain no tobacco: just extracted nicotine + plant fibres + flavourings. Chemically even simpler.

The Swedish experience: what does it prove?
Sweden is the only EU country where snus is legal (an exception negotiated in 1995). Result over 30 years:

But 'less risky' does not mean 'harmless'
A meta-analysis pooling 8 prospective studies found, in exclusive snus users:
+28% all-cause mortality vs non-tobacco users,
+27% cardiovascular mortality,
+12% cancer mortality (borderline statistical),
dose-dependent effects: worse in heavy long-term users.
Tobacco-free nicotine pouches: the new wave
Born around 2016 (Velo) and booming since 2020 (ZYN), tobacco-free pouches contain:
synthetic or extracted nicotine,
plant fibres (wood, cellulose),
flavourings (mint, citrus, lime, etc.),
sweeteners.
The absence of tobacco removes the tobacco-specific nitrosamines (TSNAs), which are among the main carcinogens in snus. Several recent studies suggest a risk profile that is even more favourable than classic snus.
The trap for young non-smokers
This is the real sensitive issue. Nicotine pouches:
taste pleasant (mint, fruit),
are discreet (no smoke, no smell),
have a 'sporty' image,
often contain high nicotine doses (4-12 mg per pouch).
Many young adults who would never smoke a cigarette take them up — and build a lasting nicotine dependence.
EU legislation: a patchwork
Quick map of the 2026 landscape:
Sweden: snus legal (historical exception).
Norway, Denmark: snus widely consumed.
Rest of the EU: snus banned from sale for a long time.
Nicotine pouches: status varies by country. Legal in Germany, the United Kingdom. Banned or restricted in some countries (France, Belgium, Netherlands, Finland for certain nicotine levels).
Poland: market booming since 2022, lightly regulated.
Snus or pouches as a quit tool?
For a cigarette smoker, switching to snus or nicotine pouches:
reduces exposure to tar and carbon monoxide,
sharply lowers the risk of lung cancer, COPD, heart attack,
but maintains, or even strengthens, nicotine dependence.

If Sweden shows snus cuts mortality, why is it banned in the EU?
A political decision in 1992, when Sweden joined the EU. Sweden got an exception. The scientific debate about whether to allow snus across the EU has been reopened several times — with no result so far. -
Do ZYN/Velo pouches cause mouth cancer?
Not significantly, based on the available data. Traditional snus has a very modest effect on some oral cancers; tobacco-free pouches, even less. -
If I switch from cigarettes to pouches, do I become 'clean'?
You drastically cut your major risks (lung, heart). You stay nicotine-dependent. If you want to go all the way to full cessation, this isn't the ideal choice. -
Is there any effect on teeth or gums?
Yes, modest but real: local irritation, gum recession. Moving the pouch's position between uses limits the irritation.
